Create a question set with its first version as a draft. Publish the version before a tool, step or grader can reference it.

Judgment questions keyed by question id. Each is { type: "boolean" | "choice" | "score", instructions, criteria }. The id is never sent to the model, so instructions and criteria carry the meaning.
