Resolve a question set

The question set exactly as an execution receives it: the current published version (or the published version named), with its current calibration. A draft-only set answers 404 QUESTION_SET_UNRESOLVED.

Judgment questions keyed by question id. Each is { type: "boolean" | "choice" | "score", instructions, criteria }. The id is never sent to the model, so instructions and criteria carry the meaning.
